Job summary The successful candidate will join the PML/Banbury Cross Health Centres (BCHC) multi-disciplinary health care team delivering primary care and community services to the Practices registered patient population.
A can do approach, is required in what can be a busy, often demanding, but highly rewarding environment.As an Advanced Nurse Practitioner or Emergency Care Practitioner, the post-holder will be expected to demonstrate advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.
The focus of the role is the provision of urgent/acute care, both on-site and possibly through visiting patients at home; with opportunities for development of the role.
You will work as an autonomous practitioner, providing high quality and timely patient-centred care to the practices patient population and to be responsible for the delivery of evidence-based clinical care and nursing services, within your scope of practice.You will adhere to professional standards, lead by example and maintain a positive, collaborative working relationship with the practices multidisciplinary healthcare team, as well as supporting the management team in the reviewing and implementation of clinical policy and procedure.
Main duties of the job To act as a self-directed practitioner who directs, provides and evaluates the highest standard of evidence based clinical care to patients in all settings within their own home, founded on agreed management care plans and protocols.To work independently, without direct supervision and perform comprehensive patient assessments based upon history taking and physical examination in a variety of clinical settings and in the home environment, prioritising workload to meet the regular unpredictable situations that may occur within this role.Demonstrate highly developed clinical reasoning and decision-making.

PML holds GMS contracts, as well as being a GP Federation representing circa 50 GP practices covering approximately 650,000 patients.We welcome applicants from a diverse range of backgrounds and circumstances and people with protected characteristics under the Equality Act 2010 Job description Job responsibilities Clinical ResponsibilitiesWork in close collaboration with the Clinical Lead for the delivery of an effective service by defining and overseeing the development of service standards in-line with CQC, NMC/HCPC guidelines and PML/BCHC Policy.Undertaking home visits and liaise with patients, carers and colleagues to assess the safety of a patients home environment, acting quickly to protect patients and staff from potential riskTo ensure that appropriate, consistent and realistic information is provided to patients and families.Be a clinical resource for the patient/carer especiallyWork within a multi-disciplinary team, to provide a coordinated, high quality and responsive service to patients.Undertake consultations with patients as an autonomous practitioner, using own clinical judgment to diagnose, treat, refer and/or discharge patientsDeliver timely, high quality, clinical careTriage, assess and diagnose patientsDevelop, implement and evaluate individual care management plans(For those with a prescribing qualification) Prescribe and review medication in accordance with PGD and within own scope of practiceMaintain complete and accurate clinical records in respect of all patient contactsMake direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols.Arrange further investigations, as necessaryLiaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.

Person Specification Qualifications Essential Relevant professional qualification Emergency Care/Advanced Nurse Practitioner qualification or equivalent.
Nurse degree or level 6 degree module in minor illness/injury assessment, diagnosis and management.
Portfolio of evidence showing achievements in practice/speciality Non-Medical Prescriber Qualification Desirable Assessor/Mentorship course Experience Essential Evidence of working within acute clinical practice Evidence of working in a community setting Minimum of three years post registration with demonstrable experience of using diagnostic skills Experience of, or a willingness to undertake advanced clinical practice Evidence of excellent clinical record keeping Evidence of clinical leadership development Ability to support patients during acute episodes of illness and to self-manage long term conditions where feasible Desirable Experience in teaching & supporting junior staff/students Skills Essential Competence in the use of IT packages including clinical databases.
Experience of clinical information systems Ability to relate to people from differing backgrounds Ability to motivate others Demonstrate positive attitude towards patients & staff Desirable Competent in databases and spreadsheets Demonstrates ability and knowledge on how to undertake audit and feedback to clinicians
