Job summary South Warrington PCN has an exciting opportunity for an experienced,hardworking and enthusiastic paramedic to join our team of five GP Practices inthe South of Warrington.
You will beable to work both autonomously and as part of a wider team, which will consistof a paramedic team, GPs, and general practice teams.
You willbe expected to work across all practice locations (on rotation).
There isalso a requirement for visiting of patients either in their own home or in acare home environment.
Therefore, applicants will have a clean driving license,along with their own car.
As part ofour succession planning, the PCNs intention is to develop paramedics to havemore of a primary care focus.
We arecurrently developing the scope for this, and therefore it is expected the newrecruits will also help to shape this role for the future.
Main duties of the job Main duties will include seeing and treating minor ailments,response for any same day requests for acute and long-term conditions, Acute visits,and Care Home visits.
You will work with other paramedics across the PCN ensuring patients are seenand treated and given the best care possible.
You will be able to:-Perform specialist health checks and reviewswithin scope of practice and in line with local and national guidance;Perform and interpret ECGs;Perform investigatory procedures as required;andUndertake the collection of pathologicalspecimens including intravenous blood samples, swabs, and other samples withinscope of practice, and within line of local and national guidance;Maintain accurate and contemporaneous healthrecords for patient.Support the delivery of anticipatory careplans and lead certain community services (monitoring blood pressure and risk of diabetes);Clinical Development Plan, (working towards).Thiswork will see our paramedics transition from the current minor ailmenttreatment model to a primary care practice model, to working on the followingtype of pathways:Chronicdisease management PCNHealth Inequality workPartof the daily practice duty team MDT modelWorkingwith teams to deliver national and local targets such as QOF/IIF/LES To manage daily surgeries comprising face-to-face consultations, telephone/emailconsultations and telephone triage work.
About us SouthWarrington PCN is one of five PCNs within Warrington, Cheshire.
We provideprimary care services to 51,763 patients, across five GP practices.
StocktonHeath Medical Centre, Latchford Medical Centre, Stretton Medical Centre,Brookfield Surgery and Lakeside SurgeryWith thecreation of the 2020/21 Network Direct Enhanced Services (DES) and theintroduction of the NHS Additional Role Reimbursement (ARRS) to support primarycare, the PCN has recruited staff under the ARRS to support and broaden ourprimary care team which allows us to provide better care for our patients.The PCN isheaded by our committed Clinical Directors, Strategic Manager, PracticeManagers and GP members.In additionto the PCN our Practices are also part of Quay Healthcare, which is anon-profit company owned by the GP Practices in Warrington.
Quay Healthcareprovide business and practical support to the PCNs and deliver additionalhealthcare services to our patients.
The PCNs contract GP provider servicesthrough Quay Primary Healthcare CIC who are responsible for providing a numberof services on behalf of the PCN; one of these services includes the managementof our PCN ARRS staff, therefore this means that you will work alongsidemembers of the PCN and Practice staff on a daily basis, with Quay Healthcare asthe lead employer.
Job description Job responsibilities Our PCNParamedic has the following key responsibilities in delivering health servicesto our patients:Workingas part of the multidisciplinary team, support the delivery, and continue thedevelopment, of the on the day patient service in the practice and PCN;Accuratelytriage, assess and prioritise patients, including face to face same day andtelephone triage, demonstrating the use of a variety of techniques to elicitthe history of an event/illness, including past medical and drug history;Proactivelyrespond to referrals and provide assessment and diagnosis at first point ofcontact, according to patients needs either in the practice or patient homes;Undertakinghome visits for the practice or PCN;Evaluatethe clinical information from examination and history taking and initiateappropriate treatment (including prescribing medications following national orpractice/PCN policy, patient group directives, national and local clinicalguidelines and local care pathways) or make necessary referrals to othermembers of the primary care team;Reviewthe effectiveness of the treatment provided, making changes where necessary toimprove patient outcomes;Communicateeffectively with patients, and where appropriate family members and theircarers, where applicable, complex and sensitive information regarding theirphysical health needs, results, findings, and treatment choices;Advisepatients on general healthcare and promote self-management where appropriate,including signposting patients to the PCNs social prescribing service, andwhere appropriate, other community or voluntary services;Communicateproactively and effectively with all colleagues across the multidisciplinaryteam, attending and contributing to meetings as required;Supportin the delivery of enhanced services and other service requirements on behalfof the PCN.Ensuredelivery of best practice in clinical practice, caseload management, education,research and audit to achieve corporate PCN and local population objectives.Participatein the management of patient complaints when requested to do so and participatein the identification of any necessary learning brought about through incidentsand near-miss events.Undertakeall mandatory training and induction programmes.Contributeto and embrace the spectrum of clinical governance.Alwaysmaintain a clean, tidy, effective working area.
Person Specification Qualifications Essential Qualified to degree/diploma level in Paramedicine or equivalent experience.
Registered with the Health and Care Professions Council (HCPC).
Completed two-year Consolidation of Learning period as a newly qualified paramedic.
has a further three years experience as a band 6 (or equivalent) paramedic.
Committed to working towards a masters level or equivalent capability in paramedic areas of practice within six months of the commencement of start date/has completed and been signed off formally within the clinical pillar competencies of the paramedic FCP/AP roadmap to practice.
Framework for Higher Education Qualification (FHEQ) Level 7 or Framework for Higher Education Qualification (FHEQ) Level 7 or working towards developing Level 7 capability in paramedic areas of practice and, within six months of the commencement complete the clinical pillar competencies of the Advanced Clinical Practice Framework.
Demonstrate excellent clinical competence (evidenced through CPD and interview).
Safeguarding and other mandatory training.
Basic life support training.
Primary Care experience is essential as you will be expected to run clinics and undertake home visits.
Desirable GCSE grade A to C in English and Maths.
Qualified to NVQ level 2 in Health and Social Care.
