Main duties of the job Toprovide a high-quality, service to the patients of Cowley Road MedicalPractice; diagnosing and recommending/prescribing treatments and medicationwhile working within defined procedures and protocols.
You will be working aspart of a clinical team, with GPs, Pharmacist, Practice nurses, and HealthcareAssistants.

Job description Job responsibilities Toprovide assessment, diagnosis and treatment at first point of contact from atriage list and act as an autonomous clinician, with the support and guidancefrom the GPs and Prescribing Paramedic.
It would be desirable to have theskills to assess children as well as adults.Proactivelyidentify, diagnose and manage treatment plans for patients at risk ofdeveloping a long-term condition (as appropriate).Diagnoseand manage both acute and chronic conditions, integrating both drug andnon-drug-based treatment methods into a management plan.Toaccurately triage and prioritise patients, including in emergency situations,demonstrating the use of a variety of techniques to elicit the history of anevent/illness, including past medical and drug history.Asrequired, to provide assessment, diagnosis and treatment for those patients whoneed to be seen in their place of residence.Toevaluate clinical information from examination and history taking and initiateappropriate treatment and / or referral, including admission to hospital ifrequired.Tosignpost appropriately to other services in the identified healthcare pathwayand to refer patients to appropriate care settings, within locally agreedhealthcare pathways.Tomaintain accurate and systematic documentation of patient interactions andclinical care provided, including adverse events.Toadvise and support members of the public on healthy lifestyles, healthpromotion and disease prevention.Abilityto support GP administrative workload (pathology results and Docman) withsuitable training and supportCommunication Toliaise and interact with multi-professional teams across organisationalboundaries, including primary care, social services and secondary careproviders according to patient needs and appropriate care pathways.
Tomaintain communication with all healthcare professionals and outside agenciesto ensure seamless, patient-centred service provision.
Toensure effective communication so that the patient receives the appropriatelevel of care and wherever possible avoid unnecessary hospital admission.
Totake an active role in maintaining expert communication with all patients,carers and relatives which meets their individual needs.
Todeal with complaints in a calm and courteous manner, ensuring that, whereverpossible, complaints are dealt with efficiently and satisfactorily inaccordance with local protocols.
Tocommunicate effectively in demanding and challenging situations, where theremay also be hostility.ProfessionalToinput to and extract information from clinical IT system.
Towork collaboratively with other members of the health care team such as theDistrict Nursing Team, Mental Health Services, staff in Care Homes, AmbulanceService, and Secondary Care etc.
Toensure that the wellbeing of patients and staff is maintained in accordancewith Practice policies.
To undertake clinical audits in line with theorganisations clinical audit plan, and to participate as required in equipmenttesting and other research.
Tocontribute to the analysis of information to inform future development of theservice.
Toparticipate and contribute to clinical meetings Toidentify risks to service provision and highlight them to the Clinical Lead.
Towork within the requirements of Data Protection legislation as it applies topatient records.Tomaintain current professional registration and indemnity.OrganisationalTooperate as an effective member of the Practice teamTomaintain accurate and systematic documentation of patient interactions andclinical care provided, including any adverse events, child protection issuesor other notifiable matters.Toprovide information to inform the evaluation and efficiency of the role and forperformance management purposes as requested.Toengage in self-appraisal and utilise reflective practice to enhance and developpersonal and organisational practice.ManagerialToensure effective time management and prioritisation of workload.Toensure the security, safe storage and maintenance of any allocated equipmentand goods.To ensure practice is within good clinical governance and based on soundup-to- date clinical evidence.
Where appropriate, to identify and assess anyrisks associated with patient care or to oneself andcommunicate these risks to the appropriate clinical manager Person Specification Qualifications Essential You must be an independent prescriber (completed Non-Medical Prescriber v300 course) Degree Nursing/Paramedic Postgraduate level education/Advanced Nurse Practitioners Experience Essential Evidence of a commitment to continuing professional development.
Experience in a primary care, urgent care Centre or emergency medicine setting.
Proven track record of delivering autonomous and patient-centred care.
Experience in managing a varied clinical caseload, including chronic and minor conditions Strong clinical reasoning, assessment, and diagnostic skills.
Proficiency in minor illness and minor injury treatment.
Excellent communication and interpersonal skills, with the ability to provide compassionate patient care.
Ability to work independently and as part of a multidisciplinary team.
IT literacy, including experience with electronic health records (EMIS) Desirable Knowledge of primary care frameworks and guidelines, including NICE guidelines and CQC requirements.
