Job summary We are looking for an enthusiastic andmotivated Band 7 Occupational Therapist Team Lead to join our highly regarded UrgentCommunity Response (UCR)teambased at Woking Community Hospital,Woking.As a key partner in thedelivery of urgent responsive services to support the frail population inhospital admission avoidance, the service is keen to offer opportunities and tosupport the development of team members to extend their skills and broadentheir scope of practice where appropriate.Although expected to work autonomouslyyou will not be alone.
The team includes Physiotherapists, Nurses, Paramedics, CommunityRehabilitation Assistants and Administrators in addition to the OccupationalTherapists; and we work closely with colleagues from the Frailty Hubs, GPsurgeries, adult social care and other community services.We provide a 7-day, 24-hour urgent responseNHS community service to patients across Northwest Surrey.
Main duties of the job Working as a member of the multi-disciplinary UrgentCommunity Response (UCR) team you will undertake assessments, clinicaldiagnosis and treatment of patients who may require management and/orrehabilitation interventions in their own environment to prevent acute hospitaladmissions and to support complex hospital discharges.
Although expected to work autonomously you will not bealone.
The team includes Physiotherapists, Nurses, paramedics and CommunityRehabilitation Assistants in addition to the Occupational Therapists; and workclosely with colleagues from GP surgeries, locality health hubs, adult socialcare and other community services.The qualities we are looking for insuccessful applicants are:Experience working as a senior clinician in admissionavoidance services.Excellent communication and time management skills.Flexible approach to work with the ability to work acrossa seven-day service.Ability to work both independently and as part of thewider multidisciplinary team.Willingness to undertake ongoing professional developmentand a commitment to providing high quality, safe and effective care that meetsthe needs of patients.

The post holderwill work as part of a multi-disciplinary team to deliver holistic assessments,interventions, and ongoing support to adults with a variety of complex needs,ensuring safe, high-quality care is provided within the community setting.
TheOT will provide a rapid assessment and short-term interventions to preventhospital admissions and enable patients to live independently for as long aspossible.
The post holder will be responsible for delivering specialistOccupational Therapy interventions within the UCR team, including conductingassessments, providing therapy, and supporting the rehabilitation ofindividuals.

Proven evidence of advanced organisational/leadership skills and autonomous practice.

Experience of delivering change management both personally and as a facilitator.

Qualifications Essential BSc/Diploma Occupational therapy or equivalent.
Registered with Health & Care Professions Council.
Minimum 5 years post graduate experience including 2 years in specialist area.
Continuing Professional Development diary including evidence of reflective practice, training and clinical experience.
Desirable Practice Placement Educators training.
