Job summary Are you a Band 4 Assistant Practitioner or Nursing Associate with a passion for making a difference?
Join our dedicated team within the South Somerset Community Mental Health Service, based in Yeovil.As a Band 4 Practitioner, you'll be working at a level beyond that of healthcare support workers, bringing a deeper understanding of the factors that influence both health and ill health.
Your primary focus will be on supporting the physical health of patients within our community mental health service, delivering care that is tailored to the needs of each individual and their family.

Person Specification Qualifications Essential Foundation degree in health care or Higher National Diploma in health care ie at Level 5 of the Regulated Qualification Framework Registered Nurse Associate on the NMC register Experience Essential Demonstrates an insight to the Scope of the Nursing Associate role as part of the multi-disciplinary team and Trust and how role will contribute to the development of our services.
