We have four branches in surrounding villages and carefor over 18,500 patients.Ourteam meets together every day for a clinical meeting over a cuppa and havelunch together whenever we can out in our award-winning garden.
Main duties of the job The role is primarily to work within our SDS (Same Day Service) Team inproviding a same day urgent treatment service and telephone triage.

Job description Job responsibilities Therole is primarily to work within our SDS (Same Day Service) Team in providing asame day urgent treatment service and telephone triage.1.ClinicalTo provide a holistic, autonomous, and clinical nursing service by attending toreferrals and self-referrals of practice patients.
Making an assessment oftheir physical, psychological and social needs, implementing an agreedmanagement plan with patients including appropriate prescribing and referral.2.Independent/Supplementary PrescribingProvide information and advice on prescribed or over-the-counter medications,side effects and interactions.Manage medicines including prescribing and administering medication andappliances for patients, as an Independent/Supplementary Prescriber, or withinPatient Group Directions.To take responsibility, as a designated independent nurse prescriber, toprescribe medicines and appliances/dressings from the relevant nationalformulary for individual patients receiving care for a condition for which thenurse has clinical responsibility.To maintain competency to practice safely as an independent/supplementaryprescriber.To comply with all legal and NMC requirements of prescribing practice.3.ResearchTo ensure Clinical Nursing activities are evidence based.When requested, to undertake an active role in carrying out research studies,independently or in conjunction with other health professionals.4.ManagementTo ensure Nursing activities remain within broad policy, objectives of theorganisation and a major role in developing policy objectives.To report, document, and evaluate the work of the Nurse Practitioner and todemonstrate its importance both to patients and the Practice.4.ProfessionalTo provide a high standard of Professional conduct and Nursing care at alltimes in accordance with the NMC.To participate in teaching Nurse Learners, medical students and other studentsas appropriate who visit.To attend relevant meetings, courses, seminars and keep professionallyup-to-date.Any other duties considered relevant to the Post.
Person Specification Qualifications Essential Registered Nurse Relevant Nurse Practitioner qualification Prescribing qualification Experience Essential At least 2 years recent experience of nursing in a Primary Care environment Minimum 5 years post-registration Management of minor illnesses or ailments Desirable Use of SystmOne clinical system Skills Essential Excellent communication skills (written and verbal) Clear, polite telephone manner Competent in the use of Office and Outlook Effective time management (Planning & Organising) Ability to work as a team member and autonomously Good interpersonal skills Problem solving and analytical skills Ability to follow clinical policy and procedure Desirable Strong IT skills SystmOne Experience with audit and able to lead audit programmes Experience with clinical risk management Personal Qualities Essential Polite and confident Flexible and cooperative Motivated, forward thinker Problem solver with the ability to process information accurately and effectively, interpreting data as required High levels of integrity and loyalty Sensitive and empathetic in distressing situations Ability to work under pressure / in stressful situations Effectively utilises resources Effectively able to communicate and understand the needs of the patient Commitment to ongoing professional development Punctual and committed to supporting the team effort Other Requirements Essential Disclosure Barring Service (DBS) check NMC registration Clinical Knowledge and Skills Essential Advanced clinical skills Clinical examination skills Management of patients with complex needs Clinical governance Quality and Outcomes Framework Broad knowledge of clinical governance Ability to record accurate clinical notes Ability to work within own scope of practice and understanding when to refer to GPs Awareness of issues within the wider health arena Knowledge of health promotion strategies Understands the requirement for PGDs and associated policy Desirable Knowledge of public health issues in the local area Awareness of issues within the wider health arena
