Job summary Are you a newly qualified GP looking to work in the heart of Norwich City, if so come and join our Norwich North Primary Care Network (PCN), where you will gain further experience and support working across three forward thinking practices within the PCN.
We are seeking someone with 2 years of CCT who has not yet held a substantive position post qualification, providing up to 8 sessions per week.
The role would consist of regular weekly sessions at all three practices, using the same clinical system (S1).
Regular clinical support will be provided by the PCN clinical director, Dr Martin Falkingham.
This will be an initial 6-month role (mandated by funding from NHSE, which may well continue).
Working as part of our multidisciplinary team, you will have the opportunity to support and care for a diverse patient group.
Main duties of the job Undertaking a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting medical problems, whether self-referred or referred from other healthcare workers within the organisation.
Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early signs of illness.
Developing care plans for health inconsultation with patients and in line with current practice disease management protocols.
Providing health education.
Recording clear consultation notes to agreed standards.
Collecting data for audit purposes and complying with QOF requirements.
Delivering enhanced services.
About us The PCN located north of the city comprises of three practices Lawson Road Surgery, Magdalen Medical Practice and Oak Street Medical Practice.
The practices are training, teaching, and research active and operate modern general practice models.
High QOF achieving and rated Good by CQC.
