Job summary As a new ARRS funded role, thePCN are looking for a dynamic, enthusiastic and forward-thinking individual, whoworks well both independently and as part of the wider Multi-Disciplinary Team(MDT).
The GP will be provided with a named supervisor throughout the durationof the post.
The funding for this role is currently guaranteed until March2025, however there may be possibilities to continue working with the Networkbeyond this date.
In line with the funding requirements, the post-holder mustbe a newly-qualified GP (that is within two years of qualification and having notheld a substantive post).
Main duties of the job The purpose of this role is to support the PCN in delivering high quality care to the patients it serves.
The role is intended to support the PCN and its practices in the delivery of their contract, which will require the post-holder to complete: Face to face and telephone consultations Home visits and Care Home visits Checking and signing repeat prescriptions Administration - dealing with queries, patient test results, referral letters NHS/private, task, emails and other paperwork and correspondence in a timely manner Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate Develop care and treatment plans in consultation with patients and in line with current Practice disease management protocols as well wider guidance such as that from NICE About us NN2 is a Primary Care Network(PCN) in North Norfolk, which is made up of five GP Practices: AldboroughSurgery, Birchwood Medical Practice, Cromer Group Practice, Mundesley MedicalCentre and Paston Surgery.
The PCN Team and Practice Teamsconsist of a number of roles, including Advanced Practitioners, ClinicalPharmacists, Pharmacy Technicians, Care Coordinators, Physician Associates, GPAssistants, Mental Health Nurses, Health and Wellbeing Coaches, and SocialPrescribers.
The PCN work with a range of local providers, including: communityservices, social care, and the voluntary sector, to offer more personalised,co-ordinated health and social care to the local population.
The PCN serve apopulation of ,000 patients.

Assessingthe healthcare needs of patients with undifferentiated and undiagnosedproblems.
This includes clinically examining and assessing patient needs from aphysiological and psychological perspective, planning clinical care accordinglyand referring patients to other services/agencies in a timely manner, utilisingrelevant guidelines.
Supportingthe delivery of advance care plans.
Recognising the need for and being able to requestand interpret investigatory procedures, including laboratory and radiologytests.
Screening patients for disease riskfactors and early signs of illness and in line with disease managementprotocols, developing care plans where applicable.
Prescribing, in accordance with relevantlocal and national guidelines, where clinically appropriate.
Supporting patients to adopt healthstrategies that promote healthy lifestyles and apply principles of self-care,including providing counselling and supporting guidance on healthier living.
Ensuring appropriate follow up ofpatients.
Understanding the wider system supportthat is available to patients, and appropriately signposting / referring towider Social Health or community organisation partners.
Recording clear and contemporaneous ITbased consultation notes to agreed standards.
