Job summary As a member of the organisation, you will have direct contact with children, and young people (CYP), and their families and will be instrumental in ensuring that plans for individual patients, and information that the organisation wishes to share with its patients, are enabled and enacted.
The post holder will have excellent communication skills, to ensure that every contact counts, through a range of media (telephone, face-to-face, IT) and will demonstrate excellent patient care through commitment to follow through of established plans to improve health outcomes.

Job description Job responsibilities To deliver and coordinate the navigator process in line with current service provisionMaintain awareness of clinical and professional developments and research where it is appropriate to do so, in order to enhance patient careIdentify, initiate and/or participate in audit and evaluation activities to enhance service deliveryMaintains accurate records electronically in a professional manner and in accordance with Trust policies, guidance and GDPR regulationsBe responsible for your own development, applying to undertake relevant training when availableHighlight risks to Clinical Leads to place on the Team Risk RegisterComment on, contribute to and interpret professional policies and procedures that are relevant and pivotal to the serviceCommunicate effectively complex and sensitive information with patients, their families and heath professionals both within and external to the organisation, adjusting to the level of communication to suit the situation Person Specification Qualifications Essential Educated to degree level or equivalent experience in a health or social care organisation Desirable Registered Health Care Professional or Social Worker Experience Essential Proven experience of working in a health or social care organisation Significant experience working with young children and families Evidence of safeguarding experience Desirable Mentoring experience Experience of using the Early Support materials Knowledge of SEND and EHCP process Skills, Abilities and Knowledge Essential Ability to work with children and young people, their families, and carers to provide holistic, evidence-based care to children Ability to work in partnership with other healthcare workers, agencies and in partnership with parents Knowledge of Norfolk Safeguarding Children Board policies and procedures Knowledge of legislation relating to children and national/local policy for children with complex needs Ability to work independently and within a team Leadership skills Decision making skills Good communication skills Observational skills IT skills Ability to teach an Desirable Group work skills Staff supervision skills Communication Essential Communicate effectively, share information and check understanding using clear language and appropriate, written materials, making reasonable adjustments where appropriate in order to optimise pelple's understanding Excellent communication skills, including verbal and written skills Personal and People Development Essential Evidence of ongoing learning and motivated to developing own skills Able to manage pressures generated by work/work environment Personal Attributes/Behaviours (linked to the Trust's Behaviour Framework) Essential Innovative approach Commitment to personal development Responds positively to change Able to identify with the Trust's commitment to safeguarding and promoting the welfare of children and young people Other Essential Ability to make own travel arrangements Able to speak the English language Enhanced CRB check
