Job summary A new opportunity has arisen for the Advanced Practitioner to join our dynamic post holder will be an experienced clinician who, acting within theirprofessional boundaries, will work flexibly to provide care for patients frominitial history taking, clinical assessment, diagnosis, treatment to evaluationof care.
The clinician in this role will be key to supporting clinical work in theIntegrated Urgent Care Service, whilst acting as a positive and enthusiasticrole model, supporting junior staff and developing our workforce.
Main duties of the job Key Responsibilitiesand Duties:Clinical Practice Assess, diagnose, plan, implement andevaluate treatment/interventions and care for patients presenting withundiagnosed and undifferentiated illness Clinically examine and assess patient needsfrom a physiological and psychological perspective, and plan clinical careaccordingly Telephone triage, calls from externalclinicians, patients and others into a range of services appropriately asrequired Assess, diagnosis, plan, implement andevaluate interventions/treatments for patients with complex needs Proactively identify, diagnosis and managetreatment plans for patients at risk of developing a long-term condition ordeteriorating outside of the hospital setting (as appropriate) Diagnose and manage both acute and chronicconditions, integrating both drug- and non-drug-based treatment methods into amanagement plan according to competency, whilst seeking opportunities todevelop knowledge around medication prescribing and administration whereappropriate.

Job description Job responsibilities Clinical PracticeAssess, diagnose, plan, implement andevaluate treatment/interventions and care for patients presenting withundiagnosed and undifferentiated illnessAssess, identify and refer patientspresenting with mental health needs in accordance with the NSF for MentalHealth if competent to do so, otherwise act as a signpost to appropriateassessments such as rapid access mental health teamsParticipate in the administration of vaccineswhere competent to do so such as seasonal influenza Promote and deliver evidence-based care forpatients presenting with a range of both minor and more significant illnessessuch as acute infections according to own expertise Meet the needs of patients presenting foropportunistic wound care Maintain competence in clinical practice inline with NMC / HCPC regulations and revalidation requirementsYou must maintain your knowledge and skillswith regards to prescribing and administration of drugs, keeping up to datewith any changes, alerts, recalls and other reasonable professional knowledgeDelivering a quality serviceRecognise and work within own competence andprofessional code of conduct as regulated by the NMC / HCPCProduce accurate, contemporaneous andcomplete records of patient consultation, consistent with legislation, policiesand procedures Prioritise, organise and manage own workloadin a manner that maintains and promotes quality Deliver evidence based care according torecognised evidence such as NICE and other national guidelines Assess effectiveness of care delivery throughself and peer review, benchmarking and formal evaluation Initiate and participate in the maintenanceof quality governance systems and processes across the organisation and itsactivities such as clinical audit, peer review and quality assessment toolsincluding risk management and incident reportingUtilise the clinical audit cycle as a meansof evaluating the quality of the work of self and the team, implementingimprovements where required In partnership with other clinical teams,collaborate on improving the quality of health care responding to local andnational policies and initiatives as appropriate Evaluate patients response to health careprovision and the effectiveness of care Support and participate in shared learningacross the practice and wider organisation Use a structured framework ( root-causeanalysis) to manage, review and identify learning from patient complaints,clinical incidents and near-miss events Assess the impact of policy implementation oncare delivery Monitor and develop the performance of theGPN team in accordance with local policies Understand and apply legal issues thatsupport the identification of vulnerable and abused children and adults, and beaware of statutory child/vulnerable patients health procedures and localguidance Have comprehensive knowledge regardingdomestic violence, vulnerable adults, substance abuse and addictive behaviour.Provide guidance and support to ensure appropriate referral if requiredLeadership personal and people developmentBe a professional advocate for the clinicalteam and the organisation in order Oversee the monitoring,stock control and documentation of controlled drug usage according to legalrequirements where appropriate Apply infection-control measures within thepractice according to local and national guidelines Advocate for policies that reduceenvironmental health risks, are culturally sensitive and increase access tohealth care for all Interpret national strategies and policiesinto local implementation strategies that are aligned to the values and cultureof general practice.
Managing informationUse technology and appropriate software as anaid to management in planning, implementation and monitoring of care,presenting and communicating information Review and process data using accurate Readcodes in order to ensure easy and accurate information retrieval for monitoringand audit processes Manage information searches using onlinedatabases and other up to date sources of information and evidence relevant tothe roleUnderstand responsibility of self and othersin relation to the Freedom of Information Act Monitor and confirm that the clinical teamare receiving and processing data and information in an agreed format Collate,analyse and present clinical data and information to the team Communicate essential financial restraintswith the team and discuss with them ideas for effective and efficient workingwithin these constraints Person Specification Qualifications Essential Masters in Advanced Practice (or equivalent) or Masters level qualifications in subjects related to primary care Experience Essential Experience of working independently within Practitioner/Specialist role.
Experience of patient contact services and/or primary care.
Recent experience of face-to-face patient care.
Understanding of patient management in PCC setting.
Sound understanding of legal/professional implications of independent practice.
Sound knowledge of local primary care services including key players.
