This is a new and substantive position in the Honiton-Ottery St Mary (HOSM)/Seaton-Axminster-Sidmouth (SAS) Cluster.
Its common to see adverts described as an exciting opportunity with no real substance behind the assertion.
However, this truly is an exciting opportunity!
The HOSM/SAS cluster is the largest cluster in all of Devon, having been formed a year ago by merging two existing clusters.
As such, we are approximately the size of Exeter in totality and more than double the size of many clusters.
This is a genuinely unique role, bringing together two specialist services within the cluster for the first time.
In terms of community health services, we could be the most advanced team in the country in terms of the combined hospice and dementia services offer.
There is no other area in Devon that has the community dementia services that we do combining Admiral Nurses, Cognitive Stimulation Therapy and Assessment and Treatment services.
At any one time, most people with dementia in the community are on our caseload.
We provide Hospice at Home services funded by Sidmouth Hospice at Home and Seaton League of Friends.
We are in the process of setting up the service in Axminster, funded by Axminster League of Friends.
The comprehensive and integrated nature of the services mean that virtually everybody in the area dies in their preferred place of death and the model of care is very highly regarded nationally.
Given the fact that the cluster has one of the oldest populations not only in England but on the planet, these services are of vital importance to the people that we serve.
We created this new management post in recognition of this and with an eye to future strategy, expansion and integration.
We are an extremely innovative and high-performing cluster.
On almost all indicators, we are the most productive/efficient cluster.
We are the only cluster in Devon to have introduced a new system of allocation which has resulted in no CRT waiting lists.
We prevent more unnecessary admissions to the acute hospital than virtually anywhere else in England and we discharge patients faster than any other area.
We do all of this by hiring the best people, creating a friendly and high-achieving culture, being innovative and adopting a systems-thinking methodology.
We are looking for a Band 7 nurse with experience in hospice care and/or dementia services.
