Job summary INTERNAL ONLY TO NORTH CUMBRIA INTEGRATED CARE NHS FOUNDATION TRUST (NCIC) EMPLOYEES ONLY An opportunity has arisen for a Band 2 Home Care Practitioner within Cockermouth & Maryport ICC.
The post holder will be based in Maryport but will be required to work across the ICC covering both Cockermouth and Maryport.
We are looking for a motivated and caring individual, preferably with some community experience, but more importantly someone who is enthusiastic about rehabilitation with older people, providing support to people who have long terms needs.
Care is delivered to ensure patients have a safe and timely discharge from hospital and prevent readmission or admission to hospital.
The Community Rehab Team is a friendly multidisciplinary team, with staff who work together to facilitate early discharge from hospital or prevent unnecessary admission.
The ability to work over 7 days is a requirement of the post.
The hours of work will be on a rota covering a shift pattern between the hours of 8 and 10 Continuing professional development is actively encouraged and as part of the team you will have the opportunity to take part in the process.

Main duties of the job As a Home Care Practitioner within the ICC, you will work with patients in their own homes to provide short-term support, aiming to maximise independence and increase confidence and quality of life.
We also provide care to those who are approaching end of life and wish to remain in their own homes to have a supported and dignified death.The team delivers care to patients which includes supporting with Activities of Daily Living, including personal care, catheter care, meal provision and support with nutrition.

Care should be delivered in such a way to promote independence where possible and as part of an ongoing plan to support the patient to remain in their own home.The team has close working relationships with all the local health and social care teams who cover the Allerdale and Copeland area, and we pride ourselves on our flexibility and ability to respond to meet the needs of our patients.ICCs aim to address the challenges of an aging population and increasing demand on health and care services by providing more out of hospital care and empowering people to take control of their health and wellbeing.
This will require new ways of working to focus on preventing ill health and ensure services are sustainable for the future.
These new ways of working will follow the same approach across all eight ICCs with flexibility to tailor services to the needs of local people.
