Job summary Weare looking for an experienced and enthusiastic candidate to join our team.This is an excellent opportunity to work with people who are affected byoffending and substance misuse within a community environment.This position requires a Nurse with appropriate and valid registration with theNMC and experience of working with individuals who use alcohol or substances.Ourclients present with complex needs and a multitude of both physical andpsychological challenges.
We offer excellent in-house training as well as thechance of further courses once the probationary period is completedsatisfactorily.
Main duties of the job Good communication skills both written and verbal are essential.
Confidence to work as part of a multidisciplinary team.Key role responsibilities include: To deliver clinical interventions in community-based hubs.
Improve healthcare access for service user, BBV testing/treatment, health care assessments and community ambulatory detox and health checks/clinical observations.
To provide and supervise the delivery of high standards of nursing care to those who use our services in accordance with up-to-date evidence based professional practice and Via policies.
Deliver general healthcare activity, including Wound Care, ECGs, and other similar activity.
Undertake basic life support/anaphylaxis training so you are confident and competent to manage a medical emergency including the administration of emergency drugs.
To support people affected by drug and alcohol use to achieve and sustain their recovery, optimising health, and well-being.
Being flexible in the hours worked will be expected of the post holder including late evenings.

Job description Job responsibilities Role PurposeTodeliver excellent drug and alcoholservices to those who use our services and theirsignificant others, includingfamily members friends and carers, to facilitate positive outcomes.Where indicated, undertake communityalcohol detoxification, support Opiate detoxification and titration in thecommunity and refer for specialist inpatient services including liaising withhospital and inpatient unit staff.Support outreach work acrossthe borough, particularly around alcohol misuse.Lead on Harm Reduction approacheswithin the service, including helping to reduce the spread of BBVs via testingand immunization schemes.Deliver general healthcare support,including Wound Care, ECGs, healthcare assessments, and other similar activity.Undertake basic life support and anaphylaxis training so you are confident and competent to manage a medicalemergency including the administration of emergency drugs.Key duties and activities of the roleProvide high quality, person centred,low threshold services, responding to the individual and diverse needs andpreferences of people and their significant others.Deliver evidence-based clinical andpsychosocial one-to-one and group interventions as part of an effectively integratedcare package.
Take an assertive approach to service user disengagement.Deliver harm reduction advice,guidance and education.
To include blood borne virus interventions, incl screening,vaccination and treatment access, and overdose prevention including naloxoneprograms, health and wellbeing screening brief and facilitated signposting andreferral to partner services, including primary care.Enable people to access education and employmentservices and health, wellbeing and recovery support activities and to signpostto other available services.Support students, volunteers, peermentors and service user representatives to take an active part in Viaservices.Identify risks and take appropriateaction in accordance with local, national and organisational Safeguardingpolicies, contributing to multi-disciplinary review meetings.Maintain accurate and timely recordsusing the designated case management system in compliance with Via InformationGovernance policies and procedures.Work flexibly, including across multiplesites and peripatetically when required.
Person Specification Experience Essential Passion for and understanding of working with substance misuse services or with other socially excluded or vulnerable groups.
With a proven ability to meet deadlines, record information accurately and in a timely way and to work calmly under pressure.
Great team working, communication and interpersonal skills.
Demonstrating a highly cooperative approach to supporting colleagues and the whole team to deliver Service objectives, as well as in working directly with people and their significant others.
Demonstrable ability to deliver an excellent quality level of service to people with complex needs.
This includes the ability to work with people on a one-to one basis or to facilitate group sessions, to be able to complete assessments, care and support plans, risk assessments and to complete case recording to a high standard and in a timely way.
Good IT skills including use of Microsoft Office programs and digital meeting platforms and Case Management Systems like Nebula, System One or Theseus.
A demonstrable understanding of the Health and Social Care regulatory requirements of the Care Quality Commission - CQC, National Institute for Clinical Excellence standards for Health and Social Care and any other relevant bodies.
Demonstrable commitment to working in partnership with other professionals such as criminal justice agencies, community groups and other stakeholders to achieve good outcomes for people.
The ability to communicate effectively, both verbally and in writing, in different settings, written, spoken, presented, etc and to different groups of people.
Qualifications Essential Registered nurse - RMN, RGN or Learning Disabilities Nurse - with a current active PIN number.
