Job summary We are looking for a talented and motivated person to join our primary care team.
The successful applicant will have excellent communication and team-working skills, be flexible and hard-working, and will work with patients and clinicians in undertaking administrative and patient-facing tasks.We are currently two practices looking to merge into one, and are also currently in the process of starting an exciting new Primary Care Network which will cater to patients in the Doncaster area.
Although the position is initially for 4 months, we envisage this to be a permanent role.
Main duties of the job The main duties of the role will be in assisting our reception team with their daily tasks, and undertaking other administrative tasks at clinicians' direction.
About us We are a small practice currently planning to merge with another small local practice, both in Doncaster.
There are three sites between the two practices, situated in Bawtry, Blyth and Cantley, and we look after a more elderly, rural population of patients.
We have a relatively small team that we are looking to expand.
Job description Job responsibilities The following are the core responsibilities of the management of medicines.
There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Key Responsibilities: 1.
Reception Assistance: Provide a welcoming and efficient reception service to patients and visitors.
Manage patient appointments, enquiries, and administrative tasks with professionalism and courtesy.

Care Navigation: Direct patients to the most appropriate healthcare services based on their needs, using care navigation protocols.
Ensure patients are informed about available services and how to access them, both within the practice and externally.

Patient Interaction: Engage with patients to understand their needs and provide guidance on accessing the right care.
Address patient queries and concerns, ensuring a positive and supportive experience.
4.
Collaboration: Work closely with GPs, nurses, and other healthcare professionals to coordinate patient care.
Participate in team meetings and contribute to the development of care navigation strategies.
5.
Administrative Support: Assist with administrative tasks, including updating patient records and managing correspondence.
Ensure all documentation is accurate and complies with practice policies and procedures.
6.
Communication: Maintain clear and effective communication with patients and the healthcare team.
Provide information and updates to patients regarding their care and appointments.
Person Specification Qualifications Essential GSCE Grade A to C in English and Mathematics Previous experience in a similar role.
Experience of working in the NHS.
Desirable Any appropriate health & social care qualifications.
