Job summary The Chislehurst Medical Partnership consists of two busy doctor's surgeries based in the heart of Chislehurst.
We are looking to recruit a part-time care navigator to join our existing friendly, hardworking team.We have recently reviewed the role of our receptionists and think that job title 'Care navigator' more appropriately reflects the scope of the role.This is a part time position for the below hours and days based at our Woodlands site:Monday 8am - Wednesday 1pm - Friday1pm - We are looking for this role to start as soon as reasonably possible.As part of our recruitment process, candidates will be subject to a second stage interview in the form of a trial shift before an offer is made.We are looking for someone who is motivated to provide the best service and;Recognises the importance of patient services and value every patient and their needs.

Main duties of the job To manage the flow of patientsarriving at the reception desk, for appointments, prescriptions or queries.
Taking calls from patients anddiscussing symptoms to triage their medical needs, actively signpostingpatients to the most appropriate service.
Receive and make telephone callsas required.
Divert calls and take messages, ensuring accuracy of detail andprompt appropriate delivery.
Making appointments on EMIS, byprocessing appointment requests for today, future appointments from patients.
Dealing with patient enquiriesefficiently and courteously.
Provide administrative support toclinical team/practice team via tasks.
Receiving specimens from patients.
Keeping reception and workingareas tidy, ensuring prescription paper is distributed to rooms as necessarydaily and locking this away again at night.
Regularly checking the receptionemail inbox Taking requests for home visitsand ensure they are recorded and processed correctly.
To offer general assistance tocolleagues/practice team, communicating with the clinical team to completerequests and resolve problems.
Maintain and manage patient records ensuring information is accurate.
Safeguard patient privacy and confidentiality.
Providingholiday and sickness cover.
Open/close the practice as andwhen necessary ensuring the premises are prepared in readiness for the morningsession.
Anythingelse that is custom and practice to enable the smooth running of the Practice.
About us Chislehurst Medical Practice is part of the newly formed Chislehurst Partnership created in July 2022 with the merger of The Woodlands Practice and Chislehurst Medical Practice.
Both are high performing, well established GP practices in Chislehurst who are passionate about providing a friendly, supportive and caring medical service to our population.
The Practices hope to move to a purpose-built medical centre in the future but for the time being are operating from two premises.We have a combined patient list size of circa 25,000.
We are a 9 Partner partnership with a team of Salaried GPs, Advanced Nurse Practitioners and a Nursing team.
