Job summary Southwell Medical Centre is the employer for this role onbehalf of Newark PCN.
Newark PCNserves a patient population exceeding 79,000, comprising seven GP practices,some of which are accredited training practices.
The PCN spans both rural andurban areas, with individual practice list sizes ranging from approximately5,000 to 22,000 patients.
All practices use SystmOne.Hours of Work: Full-time/Part-time positions available, typically hours per week.
This includes in-hours and some out-of-hours responsibilities, depending on PCN requirements.Location: GP practice(s) within the PCN, with opportunities for remote consultations.
Requirement of travel between different locations within the PCN.Team Structure: Working within a multidisciplinary team consisting of GPs, practice nurses, healthcare assistants, pharmacists, social prescribers, and administrative staff.Environment: The role involves both clinical and administrative work which are based on the clinical system (SystmOne) which is used by all seven practices, balancing direct patient care with involvement in broader PCN initiatives.Professional Development: Regular opportunities for CPD and support in maintaining professional qualifications through protected learning time and access to NHS education resources.Funding is only available for this role until 31st March 2025 at present although this may be subject to change.
Main duties of the job This role offersa unique and exciting opportunity for a newly or recently qualified GP to gaininvaluable experience across a diverse range of practices within Newark PCN.The successful candidate will rotate through different practices, spendingdedicated blocks of time in each, allowing for full integration into the teamsand the opportunity to understand the varied operational dynamics of eachpractice.
As a newly or recently qualified GP, you will benefit from dailysupport from experienced GP colleagues and will be appointed a dedicated GPsupervisor within the PCN, ensuring ongoing guidance and professionaldevelopment throughout your transition into independent practice.

You will be expected toparticipate in Practice activities including clinical and staff meetings,audit, research, and quality improvementactivities, contributing to the continual enhancement of patient carestandards.
About us Newark PCN employs a diverse range of additional healthcare professionals, including Clinical Pharmacists, First Contact Physiotherapists, Nursing Associates, and Social Prescribers.
The PCN also benefits from a well-established Care Homes team, led by an experienced Advanced Nurse Practitioner (ANP), and operates a local home visiting service to further support patient care.
Job description Job responsibilities Daily Clinical Responsibilities: Patient Consultations: Conduct face-to-face, telephone, and video consultations, providing assessments, diagnoses, treatment plans, and referrals where appropriate.
Manage a broad range of acute and chronic illnesses, ensuring that care is delivered in line with evidence-based guidelines and best practices.
Prescribe medications safely, managing repeat prescriptions and working with the wider PCN team for prescription reviews.
Chronic Disease Management: Participate in the management of chronic diseases, such as diabetes, hypertension, asthma, and COPD, ensuring that patients receive optimal long-term care.
Collaborate with practice nurses and other healthcare professionals to deliver structured care plans for patients with long-term conditions.
Preventative Care & Screening: Promote preventative healthcare measures, including immunizations, cancer screenings, and lifestyle interventions.
Identify opportunities for early intervention and disease prevention through screening, lifestyle advice, and health education.
Referral & Coordination of Care: Refer patients to specialist services when required, ensuring a seamless transition between primary, secondary, and tertiary care services.
Work closely with multi-disciplinary teams within the PCN to provide coordinated care for patients, especially those with complex or multiple health conditions.
Health Improvement Work: Health Promotion: Participate in health promotion initiatives to improve the health of the local population, including mental health awareness, smoking cessation programs, health screening and weight management support.
Refer patients to statutory and voluntary agencies as appropriate.
Contribute to PCN-led and practice-lead population health management strategies, helping to address health inequalities within the community.
Public Health & Community Engagement: Engage in public health initiatives aimed at addressing the social determinants of health and reducing inequalities in access to care.
Collaborate with local health and social care services, voluntary organizations, and community groups to enhance patient care and well-being.
Quality Improvement: Actively participate in audit, research, and quality improvement activities, contributing to the continual enhancement of patient care standards.
Implement evidence-based interventions to improve patient outcomes and reduce healthcare system inefficiencies.
Person Specification Qualifications Essential Full registration with the General Medical Council (GMC) and a license to practice in the UK.
Membership of the Royal College of General Practitioners (MRCGP) or equivalent.
Commitment to meeting the requirements for annual revalidation.
Desirable Additional qualifications in areas such as public health, mental health, or chronic disease management.
Personal Attributes Essential Compassionate and patient-focused with a commitment to delivering high-quality care.
Excellent communication skills, both with patients and the wider healthcare team.
Adaptability and flexibility in responding to the evolving needs of the community and PCN.
Organized and efficient, with the ability to manage a diverse and busy caseload.

Experience Essential Experience of managing a wide range of medical conditions.
Strong clinical decision-making and problem-solving skills.
Ability to work independently as well as part of a multidisciplinary team.
Competent in using electronic patient records and digital health platforms.
Must not have been substantively employed as a General Medical Practitioner in general practice previously.
Desirable Experience in public health or population health management.
Leadership experience in clinical settings or involvement in quality improvement projects.
