Job summary The Children's Clinical Case Coordinator role sits under the Surrey Heartlands Continuing Health Care Team and works closely and collaboratively within the immediate team and external agencies, building working relationships with all including the patient and their families and carers.
Primarily the role involves the assessing of individuals for Children's CHC eligibility, writing of a robust and comprehensive assessment, Case Management, liaison with multi disciplinary agencies and the gaining of all relevant information and evidence in support of the final recommendation.The Children's Case Coordinator will have access to support and training in all aspects of CHC in order to improve their knowledge and skills.
Main duties of the job Once a patient is assessed as being eligible for Children Continuing Care.
Our team commissions a package of care that is arranged and funded which could be:1. solely by the NHS for individuals who are not in hospital, but who have complex ongoing healthcare needs 2. with our Local Authority and education will be responsible for the assessment and case management of Children Continuing Care will use your existing and new knowledge and clinical skills to ensure that outcomes are determined in a consistent manner to ensure effective application and interpretation of the National Framework for NHS Children Continuing will work closely with the team, hospitals, community services, local authority and SEND to maintain and improve systems and processes to facilitate a smooth referral, assessment and review process to support working is what we offer.
However, we have several offices throughout Surrey Heartlands ICB, which can accommodate working from the office if required.
Ideally you will live within the Surrey Heartlands geographical area, as you will be expected to travel across Surrey for assessments and travel to your induction and meetings as required by the service.
Our main Headquarters are in Epsom and Woking.
Considering a 20- 25 mile radius would be advisable.

Person Specification Knowledge, Training and experience Essential Experience of assessment of Health and social care needs- care planning and coordination.
Ability to work effectively under stressful conditions in a rapidly changing environment.
Experience of communications with a variety of stakeholders.
Working knowledge and understanding of Safeguarding A good understanding of the health and social care environment and roles and responsibilities within it.
