Job summary We are seeking a motivated Extended Clinical Practitioner (ECP), Nurse or AHP to join our Urgent Community Response (UCR) and Frailty Virtual Ward (VW) team delivering care to patients closer to home and preventing avoidable hospital admission across North Hampshire.
The ECP will deliver specialist practice in UCR working closely alongside the Consultant Practitioner, Senior Clinical Practitioner, Advanced Clinical Practitioners, Trainee ACP's and other ECP's supporting the delivery of care in the home settings of our population.
The ECP will develop strong working relationships with the wider systems teams which may include community geriatrician, specialist doctor, GPs, and other MDT colleagues.UCR focuses on preventing avoidable hospital admissions for patients with acute ill health conditions by providing advanced clinical assessments that place the person at the core, within 2 hours of referral.
This includes implementation of treatment and appropriate monitoring whilst in crisis.
The frailty virtual ward pathway supports patients to receive ongoing care that they may receive in hospital and prevents them from deterioration that would require an admission.The service operates 0800-2000 7 days a week, 365 days a year.We are happy to discuss flexible working but would expect as a minimum working one weekend in four.
Main duties of the job To provide a high standard of complex, enhanced care for patients, using enhanced levels of clinical judgement, skills and knowledge.
Extended Clinical Practitioners will consult with patients, their family, carers and the multi-professional team to undertake assessments of patient need and devise and evaluate complex care plans related to their field of expertise.
They critically evaluate and analyse clinical problems using their expertise and clinical knowledge, seeking out and applying relevant evidence, enhanced clinical assessments, diagnostics, interventions, and equipment to make clinical decisions.Extended Clinical Practitioners deliver complex clinical care in the context of continual change, challenging environments, different models of care delivery, innovation and rapidly evolving technologies using critical analysis and their underpinning knowledge to manage complex interventions.
They recognise and work within the boundaries of their practice, knowing when and who to refer patients to.
They may delegate work to other members of the multidisciplinary team and take accountability for the delegated activity.

Person Specification Qualifications Essential Appropriate professional qualification (nursing or allied health professional) that allows registration with the NMC or HCPC Professional UK registration NMC / HCPC Degree level award Independent non-medical prescribing or pharmacology level 7 profession relevant qualification.
Holds level 7 Clinical History Taking & Physical Examination Holds level 7 Diagnostics and Decision making Able to demonstrate a good command of the English Language both written and verbal Computer Literate in both data entry and communicating via IT - emails and report
