Job summary The Enhanced Care Practitioner will support in the leadership and management of the Enhanced Care Team.The newly created Team has 30 members of staff and and is a provision offered to support patients that can challenge on a 1:1 basis.The successful candidate will also provide indepth assessments for patients utilising the service to ensure they are receiving the correct support Main duties of the job To provide clinical expertise in developing strategies to support clinical management of the patients and to support value added Enhanced Care which is therapeutic for individual patients and to support ward teams in implementing and monitoring the effectiveness of interventions Ensure the appropriate application of assessment and treatment tools that will ensure an appropriate level of intervention so that patients who present with dementia or delirium receive the most appropriate specialist care.
Develop appropriate care delivery plans, with ward staff to manage complex and difficult behaviours Demonstrate appropriate expert practice and specialist advice in the assessment, planning, implementation and evaluation of care to ensure a patient oriented approach to the delivery of care and to meet national standard Effectively liaise with other professionals such as complex case management to ensure a co-ordinated provision of specialist services to patients to assist discharge planning in intermediate care Undertake/support Mental Capacity and Best Interest To support ward managers and matrons in delivering the safe, effective and efficient use of staff resource accordance with Trust wide Policies and Procedures.
To undertake delegated management of Enhanced Care Support workers.
To include roster management, supervision, appraisal and allocation of work location.
About us As the band 6 Enhanced Care Practioner, you will work as part of the ELHT safeguarding team.
You will work closely with the enhanced care lead nurse, learning disability team and wider safeguarding team to deliver safe, personal and effective care.
Job description Job responsibilities To provide clinical expertise in developing strategies to support clinical management of the patients and to support value added Enhanced Care which is therapeutic for individual patients and to support ward teams in implementing and monitoring the effectiveness of interventions To have a working knowledge of the ELHT Standard Operating Procedure for Enhanced Care and to monitor and support compliance with enhanced care procedure and its component parts.
Ensure that the care being provided has appropriate safeguards to protect the adult at risk from abuse, and ensuring that the process offers the adult choice and support to make decisions about their safety and well-being.
Ensure the appropriate application of assessment and treatment tools that will ensure an appropriate level of intervention so that patients who present with dementia or delirium receive the most appropriate specialist care.
Develop appropriate care delivery plans, with ward staff to manage complex and difficult behaviours.
Act as a role model, demonstrating high standards of care and providing clinical leadership in the care of patients with cognitive disturbance such as dementia and delirium within clinical areas.
Demonstrate appropriate expert practice and specialist advice in the assessment, planning, implementation and evaluation of care to ensure a patient oriented approach to the delivery of care and to meet national standards Ensure that patients' receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines.
Effectively liaise with other professionals such as complex case management to ensure a co-ordinated provision of specialist services to patients to assist discharge planning in intermediate care Develop local, national and international networking and develop expert level of knowledge in the patient group.
Establish, develop and maintain effective working relationship between clinicians, practitioners and managers within the specialty and across the Trust With the support of the Safeguarding Adults lead actively participate in safeguarding investigations involving patients who have Dementia.
Undertake/support Mental Capacity and Best Interest Decision Making when applicable.
To develop training resources relating to Enhanced Care and to deliver to groups of staff To support ward managers and matrons in delivering the safe, effective and efficient use of staff resource accordance with Trust wide Policies and Procedures.

COMMUNICATION Ensure a good method of communication with all clinical areas and to encourage the exchange and implementation of new ideas and evidence based practice.
Utilise highly developed communication skills that are consistent with peoples level of understanding, culture, background and preferred ways of communicating to effectively impart information to patients and their carers/ families and in situations where there may be barriers to understanding and / or acceptance.
Communicate with patients, formal and informal care providers and partner professionals and agencies in circumstances that may be highly sensitive and contentious including the safeguarding of vulnerable adults, impaired mental capacity and degenerative long term conditions.
Demonstrate sensitivity and empathy when communicating with patients and their relatives/ carers regarding their highly complex and changing needs.
Remain professional and calm and act as a role model in situations of conflict, seeking support from others in keeping with organisational systems and policies.
Ensure written communication complies with all relevant legal professional and organisational requirements.
Communicate and collaborate across the integrated teams including discharge facilitations team, intermediate support team to facilitate timely patient/carer interventions and support and the effective transfer of care where this is deemed appropriate.
To provide support and advice across all Directorates in matters relating to Enhanced Care disseminating information as requested/necessary though appropriate channels.
To build relationships with MDT members across the directorate and senior management team To us effective communication within the team for allocation of duties.
To liaise with Clinical Site Manager, Security Team and Duty Matrons with regard where staff have been allocated.
To liaise with ward areas where support has been provided.
To give ward areas feedback when requested support has been declined or withdrawn, and the rationale behind this decision.
TRAINING AND DEVELOPMENT Work with line manager to identify own training requirements Undertake RRN approved training Ensures compliance with all mandatory training Participates in annual appraisal Participates in monthly one to ones with line manager To provide teaching on and individual and group basis in clinical practice and in the classroom (or on Teams) Maintains professional portfolio and ensures prep requirements are met to enable continued professional registration o Foster an environment conducive to learning, enquiry and research.
o Ensure the provision of a supportive and challenging environment for students from all professions o Contribute to the production of clinical procedure guidelines.
o Actively participate in health promotion and health awareness initiatives through the delivery of education programmes for patients, relatives and carers.
o Keep up to date with current national developments in clinical practice and policy.
o To provide an innovative and progressive attitude to the continual improvement of patient care through evidence-based practice and research.
Person Specification Qualifications Essential Registration with NMC Evidence of post registration education of level 6 relating to the field or equivalent experience as evidenced in their portfolio Evidence of ongoing continuing relevant professional development and Experience of team leadership Desirable Leadership qualification Degree or willingness to work towards a degree related to Dementia Teaching qualification Experience Essential Experience and expertise in the assessment and clinical management of patients with dementia/delirium Experience of MDT working Experience of teaching Desirable Experience of quality improvement Knowledge skills and competencies Essential Excellent clinical skills Relevant specialist skills and abilities for the role.
