Job summary District Nurse Team Lead-12 month SecondmentAn exciting opportunity has arisen for a highly motivated, enthusiastic District Nurse with a Community Nursing Specialist Qualification to take up the post of Band 7 District Nurse Team Leader within the Community with Core Clinical Services (CCCS) Division working across the Dudley Borough.
Main duties of the job The post-holder has overall accountability for the District Nursing Team within their Primary Care Network (PCN), and responsibility for the ongoing assessment, development, implementation and evaluation of programmes of care for patients on the District Nursing caseload.

Job description Job responsibilities Please refer to the attached job description for full details of the main duties and responsibilities that the post holder is required to undertake Person Specification Physical Skills Essential Must be able to commute to meet the demands of the role Experience Essential Evidence of leadership / management qualities Evidence of teaching / supervision skills Demonstrates up to date evidence based clinical trends in relation to community nursing Experience at Band 6 in community nursing Desirable Operational experience in the delivery of disease management/Long term conditions Knowledge and Training Essential Up to date knowledge of current Government and Local imperatives and how these translate into local practice Computer literate - ability to use software programmes designed to maximise the contribution to the post Evidence of collaborative working with multi-professional colleagues Knowledge of budget management Knowledge of Governance and Incident Management Desirable Demonstrable contribution to practice developments in primary care / disease management/long term conditions Education & Qualifications Essential First level Registered Nurse First level qualification in District Nursing / specialist Practitioners qualification in District Nursing Evidence of formal Leadership / Managerial Training and Development Evidence of commitment to life-long academic learning Nurse Prescriber Evidence of Continued Professional Development Mentor or practice based learning training Communication and Relationship Skills Essential Ability to communicate highly complex information where there may be barriers to understanding Communicate effectively across all levels of the organisation Motivated - able to motivate self and others to deliver a quality service Analytical and Judgement Skills Essential Ability to exercise independent judgement Ability to analyse data and present information to various audiences Forward thinking - to identify opportunities for improvement in service development Planning and Organisation Skills Essential Can work on own initiative Can work as part of a team Flexible - to meet the demands of the service Desirable Ability to implement programmes work leading service changes Equality, Diversity, Inclusion and Trust Values Essential Able to provide safe, caring, and effective services Values and behaviours that reflect the Trust values of Care, Respect and Responsibility Commitment to creating a diverse and inclusive workplace that is free from discrimination and where people feel they belong and their contribution is valued
