You will work closely with the Discharge Hub staff, and other partnerships to ensure a safe and efficient discharge pathway to the community.
With minimal supervision and in collaboration with the multi-disciplinary team, the post holder will be accountable for the assessment, development, implementation and evaluation of programmes of care for a defined caseload of patients working within the professional standards held in the NMC Code of Professional Conduct.For further information on the role please click the link below to view the job descriptionKnowledge, training and/or experience required to do the job:• Post holder will be a Registered Nurse with current NMC registration.• Substantial post registration experience relevant to the specialty• Working knowledge of General Hospitals Division policies and Procedures.• Ability to keep relevant skills / knowledge updated and documented.For further information on the requirements for the role please click the link below to view the person specificationHours: 1st post: 22.3 hours permanent 2nd post: 27 hours permanentThe Discharge Lounge is open Monday to Friday from the hours of 0800-2000 hours.
