Job summary As part of the multidisciplinary team, the post holder will be responsible for supporting the Central crisis response Teams, to deliver urgent health and social care to people in crisis and at risk of hospitalisation.
They will also deliver support and engage in supporting patients within the Hospital at Home service.
The Crisis Response Team receives direct referrals from NWAS (Northwest Ambulance Service) as well as Acute care, Primary care, and Community healthcare professionals.
Patients accepted by Crisis Response will be held for 48 hours before being referred onto the most appropriate onward services, if required.
The Crisis Response Team is a 7-day service operating from 08:30 -- 22:00.
The post holder will hold their own caseload of patients and follow them up after crisis intervention to signpost and provide social prescribing, to provide support within the community and help to avoid any further admissions to hospital.
The post holder will be able to support discharges from hospital completing welfare/environmental checks to complete functional assessment and identify any needs to ensure the patient is safe at home and report any concerns to the senior clinician.
These patients will have been assessed by a qualified member of the MDT in the hospital, with a risk assessment completed prior to referral to the AP welfare check caseload.

Person Specification Qualifications Essential Foundation Degree in Health & Social Care NVQ Level 3 in Care Experience of working within a community team with a high level of autonomy Current or prior experience of working in health, therapy and/ or social care setting Excellent organisational skills Communication and interpersonal skills Ability to work on own initiative and prioritise workload nderstanding of evidencebased practice and up to date knowledge of relevant literature Evidence of clinical skills competencies Ability to demonstrate empathy and be sensitive to diverse individual needs Ability to manage own case load and work without direct supervision Ability to work as part of and contribute to a multidisciplinary team An understanding of the importance of confidentiality Flexibility and ability to adapt to changing needs of clients, team and service needs Ability to work safely and reliable in both unit setting and clients homes.
