Job summary The Fenland Adult Locality Team, based in Wisbech, works with adults in the Fenland area who suffer with severe and enduring mental health.
The role will involve holding a caseload of patients with differing mental health conditions.Your role will support patients to move through their recovery journey.
As a CPN, you will be part of the nursing team, including administering depots and holding patients on Clozapine.
You will work autonomously whilst working as a fully functioning MDT.As the smallest locality team in the Trust, we pride ourselves on having a healthy work environment, where you will be encouraged to deliver high quality practice, and you will be fully supported to continue your CPD to enhance your practice and skills.
Main duties of the job To take on the role of care coordinator for clients and provide leadership to support others, to ensure that full assessment of individual care needs are undertaken, that care is planned, implemented and reviewed on a regular basis.To participate on the duty rota.To act as a role model and promote effective teamwork across the service to meet individual clients needs.To ensure that the Mental Health Act is adhered to at all times, and to work within the boundaries set out by the Trust policies and procedures.
