Main duties of the job The main duties are to work with District and Community nurses, who will lead the team and delegate wellbeing care plans, to provide domiciliary support for patients having palliative care at home.
Support with personal care, mobility, pressure area care, minor wound care, prompting medication, help with eating and drinking, ensuring a clean and safe environment, promoting independence and health and wellbeing.Providing emotional support and mental health, signposting to appropriate services for palliative and end of life care.
Working proactively and independently as well as reporting back and communicating effectively with the multidisciplinary team.
