You will be flexible, thorough, and passionate about delivering high quality care to our Frail patients in the community.The Stour Community Outreach Nurse will provide clinical support to our GPs and will work closely with the doctors and other healthcare professionals to provide healthcare for our patients.
The post holder will work within their professional boundary and capabilities at all times, referring cases to GPs and other healthcare professionals as necessary.Hours are negotiable.
Main duties of the job Conduct visits to patients in either their own home or a care home environment.Assess patients with a range of acute, non-acute and chronic medical conditions.Liaise with GPs to agree diagnosis, any further investigations which may be needed and appropriate treatment.Liaise with GPs to admit patients to secondary care if they are in acute medical need.Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other relevant care providers.To provide education and training to other staff as appropriate and to encourage teamwork and the sharing of best practiceUndertakes and interprets diagnostic tests and routine clinical procedures, including but not exclusively ECG, blood pressure monitoring, blood sampling etc.Works within all relevant practice policies and procedural guidelines infection control, chaperoning, risk management, etc.Assess and plan specialised programmes of care and care packages , referral to any other healthcare providerProvide chronic disease management, includes Diabetes, Asthma, COPD, HypertensionDevelop and review Dorset Care Plans in anticipation of the patients future needs.Deliver a weekly ward round to our care home patients as part of the Enhanced Care Home serviceProvide proactive management and clinical support for our frail patients, reducing the number of unplanned admissions to secondary careMaintain accurate record keepingTo take appropriate action where any safeguarding issues are identified About us StourSurgery is a modern friendly teaching practice with 10,500 patients working inpurpose built facilities and based in the beautiful town of Christchurch Dorsetwhere you will find stunning beaches for a great variety of water sports,Jurassic coastline for those summer evening strolls or even the zip wire offthe end of Bournemouth pier for the more adventurous!AtStour Surgery we pride ourselves in offering easily accessible high qualitycare through our award winning Stour Access System.
Our nursing team offersspecialised clinics in Diabetes, COPD, Asthma, Womens Health & Leg Ulcers.Weare extremely proud of our SCOT (Stour Community Outreach Team) & SOS(Stour Outreach Sister) staff who provide excellent care and support to ourvulnerable patients and those at risk of admission, support our GPs with homevisits & look after our housebound patients for their annual reviews, identifyingthose who are living with moderate & severe frailty.Wework alongside our Christchurch colleagues (Highcliffe MC, Farmhouse Surgery &Christchurch MP) to form Christchurch Primary Care Network and as part of theIntegrated Community Primary Care Services project where our team are workingwith housebound patients to reduce the risk of admission and helping with acutevisit requests.Someof the additional services we provide include onsite Phlebotomy and access toboth a MSK & Mental Health Practitioner.
Job description Job responsibilities The postholder will work as part of the primary healthcare team providing General MedicalServices to the patients of practice.Work in the community with the authority and directionof the doctors to identify and treat patients to reduce the need for admissionsto secondary care.Work closely with our GPs to advise ontreatment and health care for patients in the community.The post holder will attend home visits to include residentialand nursing homes.They will demonstrate safe, clinical decision-making andexpert care for patients.They will work collaboratively with themulti-disciplinary team to meet the needs of our patients.The Community Outreach Nurse willprovide clinical support to our GPs and will work closely with the doctors andother healthcare professionals to provide healthcare for our patients.
The postholder will work within their professional boundary and capabilities at alltimes, referring cases to GPs and other healthcare professionals as necessary.Requirements of Role Effectively communicates sensitive condition related informationto patients, relatives, and carersProvide and receive highly complex, sensitive, andsometimes contentious information and occasionally deal with situations whichare hostile or contentiousUse developed consultation, negotiation and conflict managementskills which require empathy, reassurance, and persuasive skills to overcomebarriers to acceptance.Regularly communicates service-related information topartners, the practice manager, clinical and administrative staff, patients, relatives,and carers.Builds and communicates effective working relationshipswith a wide array of statutory and voluntary organisations for the benefit ofpatient care and facilitate good working relationshipsRecognises the roles of other colleagues within theorganisation and their role in patient care.Demonstrates use of appropriate communication to gain theco-operation of relevant stakeholders (including patients, senior and peer colleagues,and other professionals, other NHS/private organisations ICB)Demonstrates ability to work as a member of the team Is able to recognise personal limitations and refer tomore appropriate colleague(s) when necessary Person Specification Qualifications Essential Registered Nurse Desirable Experience of working in primary care is desirable, but not essential.
Full training will be given to the successful applicant.
