Our interventions are exciting and dynamic, supporting patientflow through urgent response and facilitation of hospital discharge with a HomeFirst approach.
You will also gainexperience in core areas: Falls reduction and prevention work, case managementof long-term conditions including neurology, MSK, orthopaedics, respiratory,rheumatology, frailty and palliative and end of life care.
Main duties of the job The servicefunctions over 7 days between 8am and 6pm.
You will be aligned to the North,South or West hub area and we utilise a flexible working policy to accommodateneeds where possible.
A band 4 role may be offered if you await registration.Supported bysenior therapists you will be responsible for your own caseload deliveringholistic rehabilitation or delegating to others, made meaningful by takingplace in the home environment.
We arecommitted to your development, and you will work with the widermultidisciplinary team receiving regular clinical supervision, CPDopportunities and peer support.
You will develop leadership skills supportingothers including students and have opportunities to make a positive differenceto both your patients lives and contribute to service improvements across theTrust and wider partners.
