Job summary The Integrated Neighbourhood Teams are looking for a Band 5Community Nurse to join our community nursing team across the neighbourhoods ofCentral Woking and Sheerwater, North and West Woking, East Woking and South Woking.The Community Teamvisit patients in their own home as well as care homes.
We offer excellentopportunities for training and development of both your clinical and leadershipskills.
The team has experienced Senior District Nurses who will ensure thatyou have the necessary support for your personal development.
The communitynursing service works over a 7-day week to offering a variableshift pattern and includes weekend and bank holiday working.This post requires anAdult Nursing qualification.

Job description Job responsibilities Purpose Responsible for the delivery of care and for managingservice provision in accordance with national and local priorities,organisation policies, procedures and guidelines and in accordance with theNursing and Midwifery Council.Principal duties to include: Participate inassessment and care planning of patient care in conjunction with the DistrictNurse Implementsand evaluates care and treatment in agreement with the patient and as definedin the care plan escalates any deviation from the care plan or change incondition to the District Nurse / GP accordingly Communicateseffectively with patients, carers and all members of the multidisciplinary team Prioritisesdaily work in line with clinical / patient need Maintainsaccurate and legible patient records in line with professional andorganisational requirements this includes contemporary paper records andelectronic patient record system Implementsand works to ensure evidence based care / best practice Activelyparticipates and contributes to team meetings and daily handover Initiateshealth promotion activities with individuals, patients and carers Takesresponsibility for a defined caseload in the absence of the District Nurse,under the direction of the Practice Teacher / Team Leader Activelyinvolved in the development and implementation of community nursing servicedevelopments and initiatives Participatesin clinical audit and the implementation of changes to ways of working /clinical practice as a result of findings, recommendations and action plans Participatesin risk assessment and promotes the safety and well-being of patients, staffand any other persons at all times and assist in ensuring a safe workenvironment Ensurethat all complaints, untoward incidents, accidents and areas of clinical riskare identified and reported to the District Nurse in accordance with theorganisation policies Keepsup to date with professional requirements, health care developments, clinicalevidence, National Service Frameworks for practice, National and Local policyand guidelines Responsible for completing all own mandatory training withinthe required timeframes Accesses and participates in regular clinical supervision andpeer review Participate in own annual appraisal and undertakes owncontinuing professional development in order to maintain and develop knowledgeand skills Access regular 1-1 meetings with District Nurse Undertake mentorship training, facilitate the teaching andlearning experiences for all students and staff Accountable for own professional actions in line with the NMCcode and organisational policy and guidelines Monitors and orders stock levels and equipment in line withbudget and lean principles Responsible for the security, care and maintenance ofequipment ensuring standards of infection control and safety are maintained Person Specification Qualifications Essential Registered Nurse (RN) with current live pin on NMC register Desirable Degree or Diploma in relevant field or equivalent experience
