Main duties of the job The roleAs CommunityConnector focussed on brief intervention support you will play a key role indelivering the outcomes of the Reach and Connect service for older people inHaringey, providing 1to1 support be it in person, at drop ins, in a residents homeor remotely by:listening to individual priorities andaspirationshelping them to access a range of informationconnecting them to the right help fromavailable servicessupporting them to develop relationships andcommunity networksgetting individuals voice heard about thingsthat matter to themhelping to access volunteering opportunities/which may improve employability and/or community engagement.You willtherefore be responsible for offering generic and specialist information,signposting, guidance and short-term support to older people living in Haringeyto maintain their independence, remain in their own home and live well forlonger.About YouWe are looking for individuals who arepassionate about improving the lives of older people, and the who have welldeveloped interpersonal skills.Display warmth and understanding, understandthe importance of being a good team player and enjoy being a creative problemsolver.

Job description Job responsibilities Job Summary To have main responsibility towards delivery ofbrief intervention service, to ensure that those who need a short-termintensive intervention receive one to one support and advocacy whenappropriate.
To support Community Connectors in theirdelivery of outreach activities.
To signpost older people to appropriateservices, including those in the community and voluntary sector which may beable to meet their current and future needs.
To make sources of information about servicesavailable to individuals so that they are able to help themselves.
To connect people with the wide range ofcommunity activities available in Haringey and elsewhere to provideopportunities for new friendships and reduce social isolation.
Where appropriate to connect people to eachother through shared common interests and the need for mutual support.
To work in partnership with existing communitynavigators, VCS organisations and Statutory Services.
To ensure that all contacts, referrals andrelated activities are recorded on the CRM in a timely manner and before eachquarterly reporting deadline.
To provide performance information, dataanalysis and written reports as required.
To contribute to achieving the targets andmilestones necessary to meet the Contract specification and outcomes.
