Colorectal Specialist Nurse (Stoma Care)

Job overview An exciting opportunity has arisen in the colorectal nursing team for a stoma care nurse to cover maternity leave for a fixed term contract.

The team work over all OUH sites, providing high quality stoma care including community clinics and home visits, providing exceptional patient care which is underpinned by evidence based practice and OUH values.
In partnership with the patient and their carers and are actively involved in the caring process.
Manage, direct and evaluate the nursing care for a defined caseload of patients.
Communicate effective information to patients/relatives/carers and all members of the multidisciplinary team.
Developing and maintaining specialist knowledge of nursing practice relevant to the clinical area.
Contributing to continuing professional development in the department and mentoring students and other professionals.
Working proactively to ensure every hospital visit meets the needs of the patient, ensuring effective communication with the patient/family and other agencies.
Main duties of the job Be a defined point of contact for patients/carers for defined episodes/pathways of care and provide clinical care, specialist nursing advice and psychological support.
Be responsible for the assessment, planning, implementation and evaluation of patient care, demonstrating a collaborative approach through effective communication and decision making between the multi-professional team, patients and significant others.
Work collaboratively and in partnership with other health care professionals, offering advice to colleagues.
Accurately document and report to other members of the nursing team and multidisciplinary team information regarding the care given to patients, communicating changes as they occur.Identify and discuss patient needs with the team, patients, carers and the multi- disciplinary team.
Be able to plan own workload and those of others as required.
Assess health and psychological needs of individual patients and provide care to meet these needs, through a process of shared decision making.Demonstrate a problem solving approach to effectively execute planning, implementation and evaluation of individualised plans of care based on a person centred model of care.
