Job summary Individuals who have been diagnosed with Severe Mental Illness are at a much higher risk of the complications of physical health conditions (such as heart disease, diabetes, cancer and respiratory conditions) than the rest of the population.
There are many reasons why this is the case.
Supporting the Physical Health Care of these Patients in the community is the focus for our specialised team.

Main duties of the job The role involves undertaking Physical Health Checks for patients registered with Severe Mental Illness, either in a clinic setting or in the patient's home.You will be required to undertake physical observations, blood tests and an ECG if required.
Training can be provided for ECG and phlebotomy if necessary.You will take a physical health history from the patient and be able to signpost them to appropriate health interventions as and when necessary.
You will make full and accurate records of your care.

Job description Job responsibilities The undertaking of a Physical Health assessment for patients diagnosed with a Severe Mental Illness (SMI), either in clinic or as part of a home visit.The role necessitates accurate, comprehensive and contemporaneous documentation in patient records and appropriate communication with other health care professionals including the patient's GPSupporting the patient with health interventions that may be required as a result of your findings.Attending training as required and keeping up to date with developments in field of care as appropriate.Working as part of a team, adhering to NSFT values.Please refer to the Job Descriptions for further details Person Specification Qualifications Essential Diploma in healthcare with clinical experience, or relevant equivalent qualification Skills Essential Proficient in Basic PH Observations (BP, Baseline Observations, BMI) Reasonable confidence and competence in IT Desirable Additional skills such as ECG and Phlebotomy Knowledge Essential Understanding of Physical Health issues Understanding of Mental Health Issues
