Job summary This is an ideal opportunity for Doctors with a special interest in palliative medicine offering a full-time, fixed-term contract.
The post will be for 6 months to one year.
This post may be suitable for someone wishing to gain experience in palliative medicine with a view to applying for a higher training scheme in palliative medicine or a related specialty.
Main duties of the job The successful applicant will join a team of 2 Consultants and 7 junior doctor colleagues, including 2 Speciality Doctors, 2 SpR trainees in Palliative Medicine, 2 clinical fellows and 1 IMT trainee.
Opportunities for development and career progression will be supported in line with the applicants level of training and personal development goals.
The post-holder will work closely with internal stakeholders and external suppliers and be confident in building relationships with colleagues within the fundraising team and the wider hospice teams.

INPATIENTHOSPICE DUTIES AND RESPONSIBILITIESDuties include: Delivery of medical care to specialistpalliative care inpatients with consultant supervision (using a holisticapproach, addressing the patients physical, psychological, social andspiritual needs).
Work as part of the multi-disciplinary,specialist palliative care team to optimise patient care.
Clerknew admissions to the hospice through history and physical examination andexplore advance care planning.
Attendance at & participation in theweekly Consultant Ward Round and Multi-Disciplinary Team meeting.
Communicating with patients and theirfamilies pro-actively, providing appropriate information and support asnecessary.
Chairingof family meetings when appropriate.
Use of the multi-lingual advocacy serviceto aid assessment of and communication with patients & families who do notspeak English fluently.
Appropriatecommunication with health professionals outside of palliative medicine, toensure good continuity of patient care.
Timely provision of medical letters anddischarge summaries.
Participationin bereavement follow-up as required.2.
OUT-OF-HOURS / ON-CALLDUTIES AND RESPONSIBILITIES The on call component of this role comprisesa full on call rota working 1 in 6 weekdays and 1 in 4 single weekend days.
Theremay be additional on call duties available at other times.
Participatein the hospice first on-call rota, providing out-of-hours cover for the hospiceinpatient unit and community palliative care team with consultant supportthroughout.
On-call days at weekends andbank holidays are single-days non-resident 09:00-09:00 with estimated average workof 12 hours during each 24-hour non-resident period.
Must be available to attend the hospicewith 60 mins during all non-resident on-call periods.
On site on-call accommodation is available ifrequired.3.
NON-CLINICALDUTIES / EDUCATION, TRAINING & SUPPORTEducationalopportunities: Weekly doctors journal club.
Weekly doctors teaching.
Monthly Schwartz Rounds.
Coursesavailable via hospice Education Centre as per candidates training needs.Delivery of teaching: Teaching& supervision of medical students on placement at the hospice, both on theward setting and in the education centre (currently mainly virtual) Teaching on induction programme for StJosephs rotating junior doctors Largegroup teaching to year 5 Barts, The Royal London and Queen Marys HospitalsMedical School students, GPs, DNs and multi-disciplinary groups.Audit: Undertaking of clinical audit andquality improvement projects Supervision of audit projects undertakenby other St Josephs Hospice staff.Management Helping organise teaching and journal clubrotas.
Observation of work and teaching by StJosephs fundraising department Observationof work of volunteers department, including teaching on recruitment andselection process for volunteers.Working with consultants on service development projects appropriate tothe post-holders experience and areas of interest.Support:Regular group clinical supervision for specialty doctorsMonthly Schwartz rounds the post holder can attend and present as theywishFacilitated debriefing sessions about individual patients arranged onrequestSupervision: Regularclinical/educational supervision by named Consultant Availabilityof consultant support/advice daily following patient reviews or telephonecontacts with patients, carers, staff and external healthcare professionals.4.
GENERALREQUIREMENTS FOR MEDICAL STAFF To contact one of the consultants, othersenior colleague or managers, as appropriate, if there are any clinicalconcerns.
To make accurate, legible andcontemporaneous entries in the patients records and to ensure continuity ofpatient care by handover to colleagues.
St Josephs Hospice uses an electronicpatient records system.
To participate in audit, research or anyon-going developments within the hospice.
Toparticipate in teaching students, hospice staff and volunteers as required.
Toparticipate in journal club and educational activities at St Josephs Hospice.
To comply with Continuing ProfessionalDevelopment (CPD) requirements as stipulated by the Royal Colleges.
Tomaintain own professional indemnity insurance to cover work in the hospice.
To maintain confidentiality at all timesand comply with all other aspects of the General Medical Councils Code ofProfessional Conduct.
To keep a portfolio of evidence, to havean annual appraisal and write a personal development plan (PDP) in line withGMC revalidation policies and procedures.5.
GENERAL REQUIREMENTS FOR ALL STAFF To perform his / her duties according tothe philosophy of the Hospice, acting at all times to safeguard the interestsof patients and their families, and in particular to respect and live out theHospice Mission and Core Values.
To attend the induction programme.
To comply with Occupational Healthprocedures.
To attend any staff meetings, whereappropriate, in discussion with the Medical Lead.
To keep up to date and complete all statutoryand mandatory training, fire training, lifting and handling and otherhealth and safety issues.
To monitor, record and assist ininvestigations of any complaints from patients, visitors and staff, reportingthese to the Medical Lead.
To recognise the importance of, and toparticipate in, Clinical Governance activities at St Josephs Hospice.6.

MAIN TERMS ANDCONDITION OF EMPLOYMENTEducational supervisionwill be provided by one of the medical consultants.The post holder will beappraised via an arrangement between the hospice and Homerton UniversityHospital.The Responsible Officerfor this post, in relation to revalidation, is the Medical Director of theHomerton University Hospital NHS Trust.
St Josephs Hospicesupports requirements for continuous professional development as laid down bythe Royal College of Physicians and is committed to providing time andfinancial support for these activities.
There is a study leave budget for themedical staff.
Study leave will need tobe approved by the medical director or other member of the consultant team.Job planning will beundertaken and reviewed on a yearly basis with the Medical Director.NHS superannuation can becontinued subject to no significant gaps in service.There is an office forthe medical team at the Hospice.
Thereis secretarial support, internet access and email facilities.Three months notice oftermination of contract must be given by the postholder and employer.
