Job summary This post holder will provide clinical and managerial leadership for the community Discharge to Assess beds across Bedfordshire and Luton.
You will support patients being discharged on pathways 2 and 3 by managing flow and discharge processes.
You will work alongside senior system leaders, community therapy teams, social workers, hospital teams, care home staff and mental health services in order to support the pathways.

Person Specification Experience Essential Educated to masters level or equivalent level of experience of working at a senior level in specialist area Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master's level equivalent Desirable To have an understanding of the background to and aims of current healthcare policy in BLMK and appreciate the implications of this on engagement Professional Registration Essential A professional qualification at degree level / registration in adult nursing Organisational Skills Essential Ability to manage competing priorities in a busy and changing environment Desirable Project management skills Analytical Essential Problem solving skills and ability to respond to sudden unexpected demands Management Skills Essential Must be able to prioritise own work effectively and be able to direct activities of others.
