This information isremoved before shortlisting.The post holder will play a key roleacross the CHC service, working within the remit of the National Framework forContinuing Healthcare, to deliver an effective, efficient and equitable serviceto the people and populations of Sussex.Prior knowledge of CHC would bebeneficial, but is not essential for this role, as you will work within anexperienced and supportive team who will support you to develop the relevantknowledge required.
Main duties of the job However, the post holder should be a Registered Nurse, have experience of working within health or care, and will need excellent verbal and written communication skills, as you will be required to work closely with patients, families, carers and other external partners.As a Practitioner, your role willinclude completing reviews for nursing home residents who are in receipt ofFunded Nursing Care (FNC) You will work closely with nursing home staff toensure reviews are undertaken in atimely fashion and ensuring a good standard of care is being delivered.As this is a hybrid role theseassessments can be completed virtually (either from the work office or home) orwithin the community in nursing homes.
Therefore, you will need to be able totravel to support with your role.
Due to the hybrid way of working you will behighly motivated and have the ability to work independently, but also as partof a team.You will be supported to develop thenecessary skills and experience to work across the whole pathway, safely,confidently and effectively.

This will include the management of new referrals received via the CHC portal, liaising with the wider communities and other professionals as required, and progressing referrals through the CHC assessment process.The role will also include any duties commensurate with a Band 5 clinical role, such as assisting with Fast-Track referrals, case management, FNC reviews and applying the National Framework for Continuing Healthcare across Sussex.
This will be in line with the pan-Sussex standard operating processes and procedures and ensuring compliance with the National Framework for NHS Continuing Healthcare and Funded-Nursing Care.ENGAGING PEOPLE/KEY WORKING RELATIONSHIPSThe post holder will be expected to develop strong working relationships with patients and theirFamilies or representatives, members of the AACC Team, ICB staff, Local Authority colleagues,Care providers, voluntary and independent sector staff.Assisting and supporting individuals and representatives in relation to the Continuing Healthcare processCommunicate information, risks and issues, including briefings and reports to line manager or relevant point of escalation, such as Team Lead.Works closely with care providers involved in delivery of the service to CHC funded patients.Works in partnership with all appropriate stakeholders and participates in multi-disciplinary and inter-agency working in partnership with the individualRespects the professional opinions of all those involved in the assessment process and demonstrates leadership as part of the integrated system.Actively listen to people and their representatives and has the ability to work empathetically andSensitively with individuals along the Continuing Healthcare pathway and seek to treat them with respect and dignityDeliver the role in a way which is led by the individual and has the ability to work collaboratively with Health and Social Care practitioners and patients, families of patients and carers, whilst being respectful of individual beliefs and opinions Person Specification Qualifications Essential Current 1st level registered nurse / NMC registration A Professional qualification or degree in a relevant discipline or equivalent relevant work experience.
Good GSCE/O level passes in English and Maths, or equivalent Typing and ECDL or similar computer qualifications Evidence of further education and/or development.
