Job Overview: We are seeking a Band 6 Occupational Therapist to join the Community Services team at East Cheshire NHS Trust.
In this role, you will provide specialist occupational therapy services to patients in their homes or community settings, supporting their recovery, promoting independence, and improving their quality of life.
Key Responsibilities: Deliver comprehensive assessments and interventions to enhance patients' independence and well-being.
Manage a caseload of patients with complex needs, ensuring timely and effective treatment plans.
Collaborate with multidisciplinary teams to deliver holistic care tailored to individual needs.
Provide expert advice and guidance to patients, families, and carers.
Maintain accurate documentation and ensure compliance with professional standards and trust policies.
Key Requirements: HCPC registration as an Occupational Therapist.
Proven experience in community-based occupational therapy.
Ability to manage a diverse and complex caseload independently.
Strong communication and teamwork skills.
A valid UK driving license and access to a vehicle are desirable for community visits.
