Job Overview An opportunity has arisen for an enthusiastic LeDeR Reviewer to join the LeDeR (Learning from Lives and Deaths - People with a learning disability and autistic people) team in Leicester, Leicestershire and Rutland.
The post holder will be part of a small, dedicated team responsible for completion of reviews following the death of people with a learning disability and/or autism in line with the LeDeR - Learning from lives and deaths - People with a learning disability and autistic people (LeDeR) policy 2021.
The LeDeR programme is a national initiative established by NHS England in 2015 to review deaths of people with learning disabilities, learn from those deaths and take forward the learning into service improvement.
The NHS Long Term Plan makes a strong commitment to reducing the premature mortality of people with a learning disability as well as improves standards and quality of care experienced by patients and their families.
Job Description Summary (Main duties and responsibilities) To support the operational delivery of the Learning from lives and deaths of people with a learning disability and autistic people (LeDeR).
The post holder will be required to be responsible for planning and completing the LeDeR reviews, receiving reports from MDT members on the deceased persons care and treatment and utilising complex lines of enquiry to establish the facts pertaining to the key episodes of health and social care the person received that may have been relevant to their overall health outcomes.
Maintain constructive relationships with a broad range of internal and external stakeholders including but not limited to, primary care, social services, private organisations and the voluntary sector.
Always act in a professional, compassionate, and caring way, treating bereaved family members and loved ones with utmost respect and with courtesy.
Use highly developed skills to explain to families the parameters of a LeDeR review, signpost to other processes (e.g., complaints) and discuss expected outcomes, setting realistic expectations from the start.
Consider the best evidence available including the conversation with the family or loved one, conversation with the GP or GP records and other health and care data sources and using the best professional judgement and experience to determine and agree with Clinical Leads, whether a review proceeds from an initial to a focused review.
