Job summary An exciting opportunity has arisen to join our team at the Anticoagulation Service as an Anticoagulation Practitioner at Mid Yorkshire Teaching NHS Trust.
This part-time, Band 6 role will involve managing the welfare of patients on anticoagulation therapy.
Main duties of the job To manage patients on warfarin treatment, this includes; giving patients dosing information and advice, inducting new patients, training and reviewing patients on our self-testing program and undertaking audits.

Job description Job responsibilities To deliver a patient centred service which involves the consultation and computerised anticoagulant dosing service for a caseload of patients, both inpatients and outpatients.
The Anticoagulation Service is supervised by a Consultant Haematologist.To be responsible for continual and effectiveness of each patient's response to anticoagulant medication and ensuring that records are maintained to the highest possible standardEnsure post discharge patient aftercare is rigidly maintained, liaising with clinical services, contacting General Practitioners (GPs) and accurately recording all correspondence according.To be competent and confident with the use of Near Patient Testing (NPT) and Patient Self-Testing (PST) devices.Coordinate the Phlebotomy domiciliary service, liaising with clinical teams and GPs and updating clinical systems and recording activity as appropriate.Ensure workloads for each of the Phlebotomy domiciliary service is manageable and respond quickly to reallocate the workloads as necessaryThe Anticoagulation Practitioner liaises with all other specialist fields in the preparation of pre-op and post-op management, following policies and procedures.To act on behalf of the Pathology service as a whole, for all matters relating to patient facing services where direct interaction with patients is required.
