Great Western Hospitals NHS Foundation TrustAre you a highly experienced clinical pharmacist looking for a new and unique way to develop your clinical specialism, whilst playing a pivotal role in the delivery of transformational change across 3 acute trusts in a provider collaborative?
If so, we have an exciting opportunity for an enthusiastic and motivated pharmacist, with an interest in Gastroenterology, to join our Pharmacy Team at Great Western Hospital on a 12-month, fixed-term basis as an 8a Advanced Pharmacist (Gastroenterology).
You will initially focus on working across BSW Acute Healthcare Alliance to deliver some key, specialty-specific, biosimilar switch savings, but will also work towards establishing the role of a Gastroenterology Specialist Pharmacist within the Gastroenterology team at Great Western Hospital, including non-medical prescribing in outpatient/day hospital settings and supporting medicines optimisation and effectiveness in both inpatient and outpatient services.
By demonstrating the value of this post to both our Trust and the wider BSW Acute Healthcare Alliance at the end of its fixed-term, you will play a key role in influencing our future service developments and potentially the establishment of a similar permanent post.
Main Duties of the JobAct as an advanced Pharmacy practitioner for gastroenterology, planning care and identifying areas of concern related to medicine use within this specialty.Be an Independent Prescriber (IP) and continue to develop the role of pharmacist independent prescriber within the Trust.Provide a specialist clinical pharmacy service to patients in gastroenterology wards and outpatient areas in line with clinical pharmacy standards and objectives.Clinically check all prescriptions before dispensing and refer to the prescriber any unusual doses, potential drug interactions, or incompatibilities for confirmation or revision before authorising dispensing.Assist in clinical decision-making to ensure patient safety and medication effectiveness through the medicines optimisation agenda.Discuss with patients any issues about their medication, and liaise with other healthcare professionals in Primary and Secondary Care as appropriate.Review results of external audits related to patient care and develop action plans as necessary.Fulfil the role of Responsible Pharmacist in the dispensary when registerable activities are taking place, when required as part of the rota or out of hours, as defined in the relevant Trust guidance document.About UsOur STAR values - Service, Teamwork, Ambition, and Respect - are a golden thread running through everything we do. Job ResponsibilitiesWork in collaboration with system-wide colleagues to ensure best medicines value for gastroenterology.Lead and develop the clinical pharmacy service to the gastroenterology specialty within the Trust, setting and monitoring quality standards.Introduce, maintain, and develop a team and specialty-based model for clinical pharmacy service provision.Act as the point of liaison for the specialty and lead on pharmacy-related aspects of service development.Regularly meet with the Pharmacy Clinical Services Manager and Medicines Effectiveness team to develop a strategic plan for clinical pharmacy services.Provide details of output, performance, and activity of the clinical pharmacy service within the specialty at specified time intervals.Participate in trust-wide medicines assurance, governance, and specialty meetings to promote the safe effective use of medicines.Prepare business cases to support clinical pharmacy service developments within the specialty.Collaborate in the development of Integrated Care Pathways, clinical guidelines, and other required policies.Support the investigation of medication-related safety incidents and formulate action plans across the Trust.Person SpecificationQualificationsM Pharm Degree or equivalent / General Pharmaceutical Council (GPhC) registrationPostgraduate diploma (Diploma or MSc) in clinical pharmacy, community pharmacy, or similarKeeping professionally up to date including CPDNon-Medical Prescribing QualificationMember of Royal Pharmaceutical Society (RPS)ExperienceHighly experienced as a qualified pharmacistExperience working across departmental and organizational boundariesKnowledge of financial systems and structuresExperience in GastroenterologyPrevious experience as a specialist pharmacistExperience of service developmentSkillsGood communication, verbal and writtenPresentation skills - teaching and report writingUse of financial systems to generate reportsGood clinical knowledge backed up by extensive ward-based experienceAble to prioritise workload and manage time effectivelyUse of databases to collate informationDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such, it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
Salary: £53,755 to £60,504 per annum pro rata
