Job summary Ranworth PCN is part of the national PCN pilot looking at new ways of working in primary care.
In line with the Fuller report one of our projects is to create a PCN ANP lead acute service.We are looking to recruit an advanced nurse practitioner to join our multi-disciplinary team, to help us to transform and modernise pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries.
This is a permanent role and we are happy to consider applicants on a full-time or job share basis - we need cover 8:00 - 16:00, 5 days per week.We are looking for candidates with experience working in an acute on the day setting within primary care or an urgent treatment centre - applications from ACPs with relevant experience will be considered.The post holder will practice autonomously within the advanced practitioner level of competence working within the Ranworth PCN acute on the day service.
They will assess, diagnose, treat or refer patients of all ages presenting with undifferentiated and undiagnosed problems.
They will provide clinical leadership for the PCN acute service team and support and mentorship to other primary care staff where appropriate.
The post holder will work alongside clinicians within the PCN and liaising with clinician in other services.
Main duties of the job Provide clinical assessment and deliver treatments to service users who have undifferentiated health care requirements in order to support the services delivered by the primary care team and prevent unnecessary acute hospital attendances.
Utilise advanced clinical skills and competencies in clinical health assessment, medicines management, leadership and case management.
Maximise patients health and reduce risks that contribute to ill health, thereby reducing unnecessary admissions to acute services.
Participate in training opportunities for self and others within the PCN.
Support and deliver training to colleagues working within the PCN acute service.
Provide mentorship and colleagues working within the PCN acute service.
Provide line management to the team of ANPs working in the service.
About us Ranworth Primary Care Network, is comprised of Caradoc Surgery, Ranworth Surgery in Clacton-on-Sea, Green Elms Health Centre in Jaywick and Kennedy Way Medical Centre in Holland on Sea.
We have a diverse patient population, of around 32,000 and all practices work on SystmOne.
