Job summary We are looking for anAdvanced Clinical Practitioner to join our team, to help deliver high-quality,person-centred healthcare to residents in care homes across Kendal.Your role supportsthe care of residents in the care homes aligned to the practice, where yourskills and passion for the care of frail and older people will be highlyvalued.Based with the James Cochrane Practice you will work closely with lead care home clinicians in the practice and Kendal PCN to care for patients in local Care Homes.
Main duties of the job Provideexpert clinical care and management of frail and older adults in care homes.Utiliseprescriber qualifications to ensure optimal medication usage and management.Collaborateand work closely with a well-established Care Home multidisciplinary team andour neighbouring GP practices in Kendal to deliver holistic patient care.Supportand advise patients, families, and carers with your clinical expertise.Promoteand implement best practices in care of older adults, ensuring high standardsof dignity and respectRecognitionand management of palliative and end of life situationsSupportbest practice by working closely with the individual, their carers/family/NOKand communicate well with health and social care teams to ensure weconsistently deliver high quality personalised care.
About us We are a 2-site, part-dispensing practice in Kendal with around 17,500 patients, keen to attract caring, motivated, and passionate colleagues.
We are a training practice with an active frailty and care home support team.

It may on occasion involve some outof hours or flexible working to support patient need.
Person Specification Experience Essential Experience of working in frailty and the care of older adults Working autonomously in an advanced clinical practitioner role Managing both acute care needs and long-term health problems relevant to frail and older adults.
Including palliative and end of life care.
